Notice of Privacy Practices

Our Duties
MaryForward is required by law to maintain the privacy and security of your protected health information (“health information”), to provide you with this notice of our legal duties and privacy practices, to notify you if a breach occurs that may have compromised the privacy or security of your health information, and to follow the terms of the notice currently in effect.
How We May Use and Disclose Your Health Information
Treatment. We share your health information with the licensed clinicians on our platform who review your case and provide your second opinion consultation, and with other medical professionals involved in your case review.
Health care operations. We may use your health information to operate, administer, and improve our services, including quality assurance and clinician credentialing. Quality assurance activities use de-identified information where possible.
Payment. We may use your information to process payment for services you purchase from us. MaryForward does not bill insurance and does not disclose your health information to insurers or payers.
With your authorization. Uses and disclosures other than those described in this notice will be made only with your written authorization. You may revoke an authorization at any time, in writing, except to the extent we have already relied on it. In particular, most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of health information require your authorization — MaryForward does not sell your health information.
Required by law. We will disclose your health information when required to do so by federal, state, or local law, including in response to lawful court orders, and as necessary to prevent a serious threat to health or safety, for public health activities, or for health oversight activities.
De-identified information. We may use information from which identifying details have been removed for research, statistics, and service improvement, as described in the consent you provide.
Your Rights
Access. You have the right to inspect and receive a copy of the health information we maintain about you, including an electronic copy. You can view your case information at any time through your patient portal, and may request a complete copy by contacting us.
Amendment. You have the right to request that we amend health information you believe is incorrect or incomplete. We may deny the request in certain cases, and will tell you why in writing.
Accounting of disclosures. You have the right to request a list of certain disclosures we have made of your health information.
Restrictions. You have the right to request restrictions on how we use or disclose your health information. We are not required to agree to every request, but will accommodate reasonable requests where we can.
Confidential communications. You have the right to request that we communicate with you in a specific way or at a specific location (for example, only by email).
Paper copy. You have the right to receive a paper copy of this notice on request, even though it is provided electronically.
Breach notification. You have the right to be notified if a breach occurs that may have compromised the privacy or security of your health information.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer by email at team@maryforward.com with the subject line “Privacy Complaint,” or with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue S.W., Washington, D.C. 20201, by calling 1-877-696-6775, or online at hhs.gov/ocr/privacy/hipaa/complaints. You will not be retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for health information we already hold. The current notice, with its effective date, will always be posted on this page.
